Scheduling your first visit
- Get a referralAsk your primary care provider to fax a referral and recent records, including labs and imaging, to (410) 787-9405.
- Request an appointmentOnce we receive your referral, request an appointment online or call us at (410) 787-9400.
- Arrive earlyPlease arrive 15 minutes before your appointment time.
What to bring
Your visit checklist
- Photo ID and current insurance card(s)
- A list of all current medications, including doses and supplements
- Recent lab results, X-rays or other imaging reports
- Names and contact information for your other physicians
- Your co-pay, if applicable
Insurance
We participate with many major insurance plans. Please call to confirm coverage before your visit.
Prescription refills
Please contact your pharmacy first and allow 2–3 business days for refill requests. Some medications require an office visit or current lab work.
Policies
Cancellations & missed appointments
If you need to cancel or reschedule, please call at least 24 hours in advance so we can offer the time to another patient.
